Please download the form(s) required. Print the form(s), fill it up and sent it via courier or express mail to the Management Company's Head Office or the Regional Centres nearest to you for immediate processing upon receipt on any transacting day.

You may also fax the signed form(s) to the Head Office at 03-2096 1020 or 03-2089 1801 on any business day followed by a call at 03-2089 2800 to confirm receipt. It is advisable to fax us by 3:30pm to provide us sufficient time in order to serve you better. Following that, send the signed forms to the Management Company's Head Office.
